Alumni Survey

As we did for 2002, we'd like to create or update some interesting stats about the Uni '72 class, a slide show exhibit for the reunion dinner, and perhaps a digital 'Memory Book' on CD. Please fill out the Alumni Survey below as completely as possible. In the interests of privacy, ALL INFORMATION IS OPTIONAL except for your name. (The slide show and CD Memory Book will not include street address or phone numbers, but may include anything else submitted here).

This info will also be used to contact you regarding the event if we need to do so, so please include mailing and e-mail addresses for that purpose. This survey generates a data record sent to Pete Girard. Personal data from this survey will not be used on this website, except for your e-mail address (but only if you give us permission by checking a box near the end, so that we and other classmates can contact you). To limit the creation of survey records by others (who are not you), we will check middle names (use the one in the yearbook). We will also contact you by return e-mail if the survey data looks questionable (e.g. not you). You can fill out this survey more than once, the most recently received survey will be used, but no later than July 1, 2012. Please fill out this survey even if you don't think you can make it to the reunion; your classmates would like to know how you are and you can leave a message for them!
